2025 年 7 月 10 日， 艾伯維 (NYSE: ABBV) 和紐約Ichnos Glenmark Innovation, Inc. (IGI)的全資子公司 IGI Therapeutics SA宣布了一項獨家許可協議，針對 IGI 的主要研究資產 ISB 2001，該資產使用 IGI 專有的 BEAT 蛋白質平臺開發，用于治療腫瘤和自身免疫性疾病。

Ichnos Glenmark的BCMA/CD38/CD3三抗ISB2001，該抗體采用1:1:1設計，并且在Fc段引入了L234A、L235A和P329A突變，去除ADCC效應，另外其通過共同輕鏈放在輕鏈和重鏈錯配。

• 15 名接受過 BCMA 靶向治療的患者中，ORR 為 73%，CR/sCR 率為 27%。
• ISB 2001 的中位半衰期約為 17 天，這支持了降低給藥頻率的潛力。

ISB 2001 在整個劑量遞增階段表現出良好的安全性，未報告劑量限制性毒性 (DLT)。24 例患者（69%）出現細胞因子釋放綜合征 (CRS)，主要為 1 級，僅有 4 例患者出現 2 級事件。CRS 主要發生在 ISB 2001 的首次給藥，未觀察到嚴重或危及生命的病例。藥物相關的嚴重感染并不常見（4 例患者，11%），且未報告 4 級或更高級別的感染。1 例患者出現 1 級 ICANS；未報告其他藥物相關的神經系統不良事件。

CD3/BCMA/CD38三抗業界做的人不多，其實2020年有文章初步顯示，做三抗具有一定可行性。而我也在2020年5月4日介紹過一篇文章“”

首先BCMA/CD3雙特異抗體的體外數據確實不錯。

BCMA/CD3雙特異抗體的殺傷效果，跟BCMA的表達也沒有相關性。Killing capacity was independent of BCMA expression levels and only moderately affected by the immune suppressive BM microenvironment (BMSCs,Bone marrow stromal cells).

To evaluate whether clinical resistance to bortezomib, lenalidomide, and/or daratumumab has an impact on the activity of JNJ-7957, we analyzed JNJ-7957-mediated killing of BM-localized MM cells from NDMM and RRMM (newly diagnosed (ND) or relapsed/refractory (RR) MM)patients in flow cytometry-based cytotoxicity assays.

Interestingly，最后發現從經daratumumab治療的患者提取的BM-MNCs，JNJ-7957具有更好的藥效。

作者Hypothesized that the improved JNJ-7957 mediated MM cell killing after in vivo daratumumab treatment is the result of these immunomodulatory effects. 又做了這樣一個實驗，RPMI-8226 cells were treated with JNJ-7957 in the presence of PB-MNCs from these patients. JNJ-7957-mediated MM cell lysis was superior in the presence of the daratumumab-exposed PB-MNCs, when compared to the daratumumab-na?ve PB-MNCs.

確實，daratumumab-exposed PB-MNCs藥效會好一些。

作者不放心，又進一步做了相關實驗，To assess if daratumumab treatment is also beneficial for other T-cell redirecting therapies, we treated CD19+ Raji cells with blinatumomab, an FDA-approved CD19xCD3 BiTE for the treatment of acute lymphoblastic leukemia, using paired daratumumab-na?ve and exposed PB-MNCs from11 of these 13 MM patients.

Similar to our observations with JNJ-7957,the activity of blinatumomab was significantly enhanced by co-incubation with daratumumab exposed PB-MNCs, when compared to daratumumab-na?ve PB-MNCs

與CD3/BCMA雙抗結果相似。Daratumumab確實能夠提高CD3雙抗的殺傷作用。

為此作者作出了解釋，說了CD38抗體的兩個作用，一個是its classic Fc-dependent immune effector mechanisms，一個是CD38-targeting antibodies improve anti-tumor T-cell immune response by increasing NAD+ levels in T-cells. 然后又強調，Importantly, although daratumumab-refractory MM cells were resistant to the direct effects of daratumumab, our results indicate that the immunomodulatory effects ofdaratumumab are preserved in daratumumab-refractory patients and contribute toimmune-mediated killing of MM cells.

相當于沒解釋。一切靠數據說話。

那么直接聯用的效果怎么樣呢？Direct combination of daratumumab and JNJ-7957 enhances MMcell lysis by JNJ-7957 in anadditive fashion.

在經過daratumumab治療過的患者BM-MNCs，daratumumab 與JNJ-7957直接聯用藥效并未增強，但未經daratumumab治療的BM-MNCs藥效確實有明顯增強。

作者對CD3/BCMA雙抗的數據很有信心。

并且強調，In addition,our data indicate that clinical evaluation of the combination of daratumumab and JNJ-7957 is warranted.
