代謝病已經成為人類的一大敵人，從近幾年以司美格魯肽為代表的各種減肥“神藥”的火熱就可見一斑。今日，《自然·代謝》雜志發表了一篇新論文，研究者們開發了一種基于阿司匹林前體水楊酸的硝基烯烴衍生物，SANA，作為新型的降糖減重小分子藥物。

SANA可通過增強線粒體呼吸和增強脂肪組織能量消耗來減少肝脂肪變性和胰島素抵抗，而且該作用獨立于已知的脂肪組織產熱通路UCP1和AMPK。

在1A/B期臨床研究中，研究者驗證了SANA的安全性和耐受性，2周治療顯著改善了受試者的體重和血糖管理，證明SANA是一種有潛力的糖尿病治療候選藥物。

在開發新的代謝疾病藥物的過程中，研究者們注意到了硝化不飽和脂肪酸，這是一類經臨床前研究驗證的具有積極代謝作用的化合物。此前已有研究發現，它們的藥理學作用主要來自其反應性硝基烯烴基團。

通過與不同的化合物結合，硝基烯烴基團能夠在保持其代謝益處的同時衍生出不同的藥理特性。研究者嘗試了不同的組合，最終選出的支架分子是我們的研究常客，水楊酸，也就是阿司匹林的前體化合物。

大家肯定已經很熟悉阿司匹林的抗炎鎮痛作用，但未必有多少人知道，阿司匹林還能和減肥扯上關系吧。

此前有研究發現，高劑量水楊酸鹽（2-5g/kg/d）能夠激活AMPK通路并改善肥胖小鼠的代謝和體重。在臨床試驗中，也曾觀察到水楊酸對血糖相關代謝指標的改善。

本次研究提及的SANA，就是5-（2-硝基乙烯基）-水楊酸。研究者首先在小鼠中進行了藥理學測試，以400mg/kg/d的劑量口服給藥，治療8周未見明顯肝腎毒性。對飲食誘導的肥胖小鼠（DIO），SANA能夠在不影響小鼠正常生長、飲食、運動的情況下，保護小鼠少長脂肪、多長肌肉。同時，SANA小鼠也免遭脂肪肝困擾，血糖水平、葡萄糖耐量、胰島素敏感性等代謝指標也非常正常。

研究者將SANA（200mg/kg/d）與水楊酸鹽（200mg/kg/d）和二甲雙胍（300mg/kg/d）進行了比較，SANA的控糖效果更好。

通過對白色脂肪組織的蛋白質組學分析，研究者探索了SANA背后的作用機制，發現SANA顯著上調了脂肪組織的肌酸依賴性產熱和代謝循環，增強了線粒體呼吸，而且作用獨立于AMPK和UCP1。

最后，研究者招募了肥胖/超重和健康志愿者，開展了一項隨機、雙盲、安慰劑對照的1A/B期臨床試驗，采用單次/多次劑量遞增方案。試驗期間未觀察到嚴重不良反應，治療15天后所有受試者均肝功能正常，藥物不會隨時間積累在體內。

15天的治療對肥胖/超重受試者的體重產生了顯著影響，最大劑量組（200mg/12h）體重減輕了約3%，這一幅度與司美格魯肽的臨床試驗表現相當。此外，受試者的糖代謝也有所改善。

由以上結果可見，SANA具有頗大的減重/改善代謝功能，是一種有潛力的同類首創小分子藥物。其他它的后續臨床試驗結果。
