Doctor:Yes, sure. Whose order is it?

Nurse:It's Mrs Lim. I wanted to check herfurosemideorder. I've got the medication chart here. Mrs Lim'sfurosemide dosewas increased from 20 mgs to 40 mgs after theward round. Is that correct?

Doctor:Yes. That's right. Is there a problem?

Nurse:Yes, there is. You didn't write in the newdose. Also. the order hasn't been signed and there's no frequency of dose written in either. I wanted to check with you first.

Doctor:Oh yes, thanks for pointing it out. I think I was called away to an emergency when I was writing it up. I want her on 40 mgs offurosemideand it should be givenb.d. She's having a lot of breathing problems and her legs are quiteoedematous.
